Why churches use Forms for Church for this

  • Plan with a headcount

The template asks how many people are attending and leaves room for additional names and practical needs.

  • Meet people where they are

Share a link or QR code in the bulletin, or embed the registration form on your church website. Respondents don't need an account.

  • Keep your event team informed

Choose who receives new-response notifications. Emails use a secure link by default, so you decide which answers belong in an inbox.

  • Prepare your attendee list

Review responses in your workspace and export a CSV for your event team. This template collects registrations; it doesn't take payments or reserve limited seats.

Planning Center People · Pro

A next step for attendee follow-up

When your church connects Planning Center on Pro, a routing rule can send a selected respondent's name and email to People. You can also choose a workflow for the new person.

  • Map the respondent

Choose the respondent's name and email fields. Additional attendee names are not turned into separate People records or event registrations.

  • Ask permission and check matches

Your church can require an explicit Yes/No permission answer before sending. If People already has the email, delivery stops for your team to review; existing people are never changed.

  • Keep the boundary clear

The connection sends only the mapped name and email. It doesn't import your People directory or sync form responses back. This sample never connects to Planning Center.

Name-tag printing for your welcome table

Tell us how your team would like to print name tags from an iPad to a Brother label printer. We can discuss the integration work, printer support and setup your church would need. Printing, Planning Center Check-Ins and check-in labels aren't included in the form workflow.

From template to a shared link

Three steps from a ready-made template to a live form.
  1. Step 1: Start with the Event Registration template

Choose it when you create your account, or later from Templates. It opens as a draft that only your team can see.
  2. Step 2: Make it yours

Every question can be edited, reordered or removed. Add your church's logo and colors, and choose who is notified.
  3. Step 3: Publish and share

Publish to get a link and a QR code for the bulletin or screens, or embed the form on your website.
