Terms of Service

The service

Forms for Church provides tools for churches and ministries to build forms, collect submissions, and route them to their teams. You are responsible for the content of the forms you publish and for how you use the information you collect.

Accounts and organizations

An organization is administered by its owner. Owners and admins control who can access forms and submissions. You are responsible for keeping your credentials secure.

Acceptable use

Do not use the service to collect payment card numbers, government identifiers, or other data that requires protections the service does not provide, or to send unsolicited communications.

Church-directed delivery

Your church chooses the people who receive notifications and the webhooks that receive responses. Tell respondents where their information will go. Deleting a response here cannot retract a copy already delivered to one of those destinations. The planned Planning Center connection is not currently available; when offered, it will send selected information to Planning Center without importing or syncing a Planning Center directory into this service.

Billing

Paid plans are billed monthly through our payment provider, Stripe. Plan changes are handled through Stripe Checkout or the Stripe billing portal. The timing of plan changes, possible prorations, trial length, post-trial access, and refund handling require owner review before these terms take effect. Cancellation does not automatically delete your data; the retention and deletion terms in the Privacy Policy apply.

Availability and liability

Any availability commitment, warranty language, liability limit, and dispute terms require owner and legal review before these terms take effect.

Changes

We may update these terms and will note the effective date here.
