Referral Program Terms

These rules apply to the limited Forms for Church referral pilot. The pilot opens only when an eligible church owner has a referral link available in their account. A link may be withdrawn when the pilot reaches its limits. Sharing a link or a new church signing up does not reserve a reward.

Who can participate

A referrer must be at least 18, have a confirmed Forms for Church account, and own one eligible church workspace when the link is issued and when a reward is approved. The pilot is limited to people and churches in the United States. Forms for Church staff, internal or test accounts, and anyone acting for the same church on both sides of a referral are ineligible. Each link belongs to the church that owned it when issued. A church can receive no more than five approved referral rewards, even if several owners share links.

How a referral qualifies

The new church must use the referrer's unique link when its first account is created, create a genuinely new church workspace, complete its original 14-day trial, and make its first successful payment for a regular monthly Starter or Pro subscription. The payment must be for the full plan price and remain valid. A signup, published form, payment method, free trial invoice, failed payment, or test payment does not qualify. A later payment cannot replace a first payment that was reversed, disputed, or credited.

We review the account and church identities before approving a reward. Self-referrals, duplicate people or churches, shared church membership, internal or test accounts, and attempts to manipulate the program are ineligible. We may request reasonable information needed to confirm eligibility. We do not share the referred church's name or payment details with the referrer.

Reward and pilot limit

Each approved referral earns one $10 USD reward for the referrer, paid manually through PayPal or Venmo. When a referral qualifies for review, the referrer can provide their PayPal email address or Venmo username in account settings. Use an account you own and check the details carefully. Forms for Church covers delivery fees within the pilot budget; the recipient should receive $10. We will contact you if your chosen method cannot be used within that budget. Your provider's account and withdrawal terms still apply. The reward is not a subscription discount or a payment to the referred church.

The pilot can approve at most 23 rewards, with no more than five for one referring church, and total reward and delivery costs capped at $250. We review candidates in the order their qualifying first subscription payments were recorded. Eligibility review, payment reversals, and the remaining budget can affect whether a candidate is approved. A pending candidate does not reserve a place or guarantee a reward. If the pilot fills before a candidate is approved, that candidate receives no reward. We will stop issuing links when the approved reward cap is reached, but links already shared may no longer qualify new churches.

Delivery and changes

After the qualifying payment, we aim to complete review within 30 days. For an approved reward with valid payout details, we aim to send payment within 30 days after approval. Payments are reviewed and sent by our team, so they are not instant or automatic. We may take longer to resolve identity, payment, provider, or abuse concerns and will contact the referrer at their account email when action is needed. A refund, dispute, or credit note before delivery can cancel eligibility. We do not promise a reward until approval is recorded. We may pause or end the pilot for new referrals, but will review already approved rewards under these rules.

Disclosure and privacy

Whenever you recommend Forms for Church with your referral link, clearly tell the audience that you may receive a $10 reward if a church you refer becomes a paying customer. Keep that disclosure with the link in posts, emails, and messages. Our copy button includes it, but you remain responsible for making it visible in the place you share.

Our Privacy Policy explains how referral and reward records are handled. Your payout details are available to you and authorized Forms for Church administrators and are used to arrange and record your reward. PayPal or Venmo processes the payment under its own terms and privacy notice. Questions or a review request can be sent to hello@formsforchurch.com.
