Sending form responses to Planning Center
Forms for Church can add a new person to your church's Planning Center People account when someone fills out one of your forms. It only sends information outward. It never imports your People directory or syncs changes back.
Churches on Pro ($39/month), including during the 14-day Pro trial, can connect it from the Integrations page in their account.

What it does, and what it never does
What it does
- You add a routing rule to a form with the action Add a new person to Planning Center. When a response matches the rule, Forms for Church sends that respondent's name and email to Planning Center People.
- It creates a new person only when nobody in People already has that email. If someone does, the delivery stops and shows the match so you can resolve it in Planning Center. Existing people are never changed.
- You can also add the new person to a Planning Center workflow you choose.
- You can add an optional Yes/No permission question: “May we add your name and email to our church's Planning Center People records?” With it, only “Yes” adds the person.
What it never does
- It never imports or copies your People directory, forms or responses into Forms for Church.
- It never syncs changes from Planning Center back to Forms for Church.
- It sends only the mapped name and email for this action. The rest of a response and any uploaded files stay in Forms for Church.
- It makes only the limited checks needed to confirm the right account, look up an exact email match, and avoid duplicate workflow cards.
How to connect
What you need
- The Pro plan ($39/month), or the 14-day Pro trial. Routing rules are also a Pro feature.
- Admin access to your church in Forms for Church.
- A Planning Center login that can approve access to your church's People account. Forms for Church asks for People access only.
- A form with name and email questions.
Step 1: Open Integrations
In Forms for Church, open Integrations and choose Connect Planning Center.Step 2: Approve access
Sign in to Planning Center and approve access.Step 3: Confirm the account
Back in Forms for Church, confirm the Planning Center account that should receive people.Step 4: Add a routing rule
Open a form's settings, add a routing rule, and choose Add a new person to Planning Center. Map the name and email questions and, if you like, pick a workflow.Step 5: Publish the form
New matching responses are sent automatically, and each delivery shows its status.
To stop, choose Disconnect on the Integrations page. New responses are no longer sent. People already added stay in Planning Center.
